Application Checklist

WHen requesting fuel assistance, you must bring the following with you to the interview with the fuel certifier:
  1. Social Security Cards for everyone in the household.
  2. Photo Identification of everyone in house hold over 18 years, social security cards or birth certificates.
  3. Income verification for everyone in the household over the age of 18:
  1. Wages
    1. Most recent paystub if paid monthly
    2. Two most recent consecutive paystubs if paid bi-weekly
    3. Four most recent consecutive paystubs if paid weekly
  2. Alimony or Child Support
    1. DOR Report
    2. CAPIC Financial Assistance Form (contact office for form)
  3. Unemployment
    1. Benefit Award Letter
    2. Four Consecutive paystubs or a bank statement
  4. Social Security (SS/SSI/SSDI)
    1. Benefit Letter
  5. Gross Pension
    1. Gross pension letter from company providing pension.
    2. Most Recent Check Stub
  6. Foodstamps/Tafdc/EAEDC
    1. Benefit letter specifying amount received
  7. No Income
    1. Contact CAPIC
  8. Full Time Students
    1. Letter from registrar's office of class schedule
  9. Self-Employed
    1. Copy of 2009 Federal Income Tax Return including Schedule C
    2. Copy of 2010 Federal Income Tax Return including Schedule C
  1. Heat Source:
    1. Oil Company / Kerosene Company / Propane Company (with CAPIC Contract)
    2. Gas or Electric Bill
  1. Renters:
    1. Updated copy of lease
    2. Section 8 Lease / Addendim (showing rental breakdown)
    3. Public Housing Lease
  1. Home Owners:
    1. Mortgage Statement or Mortgage Interest Statement
    2. Home Owner's Insurance or Real Estate Tax Bill, or Water or Sewer Bill
    3. Rental Income (Current Lease or Letter written from landlord)
    4. Homeowners with rental property need to submit Federal taxes wiuth Scheudle C or interest on mortgage, a water & sewer bill, cost of homeowners insurance, property tax bill and rent receipt or lease for tenants.
    5. Homeowners of single homes/condo submit a real estate tax bill and the cost of the homeowners insurance if the house is paid for or a mortgage statement if there is a mortgage.
