- Social Security cards for everyone in the household.
- Recent rent receipt and a copy of your housing lease.
- Photo I. D. for the applicant.
- Income verification for everyone in the household over
the age of 18:
- If paid weekly we need four (4) recent pay stubs;
if paid bi-weekly we need 2 recent pay stubs.
- Copy of SSI, SS, SSDI, or TAFDC grants verification.
- If income is alimony, child support, pensions or unemployment,
please bring a copy of the check.
- If you are self-employed, please provide a copy of
your tax return.
- A recent gas bill, electric bill and phone bill.
- If a member of the household is 18 or older and a full-time
student you must have a letter from the school or college.
- If you are a homeowner, you must have a copy of your
mortgage statement, homeowner's insurance, tax bill and
water/sewer bill. If your house has two (2) or more apartments,
you must have copies of the rent check you receive from
your tenants.
