Paperwork you need to apply for CAPIC Head Start:
  1. Child must be three years old upon entry into the program of that school year.
  2. Birth Certificate
  3. Social Security Card
  4. Proof of address:
    • Utility Bill with name and address
    • Phone bill with name and address
  5. Proof of income:
    • Four pay stubs for each person in the family that works.
    • Welfare letter.
    • Social Security Income (SSI) letter.

      • Families must meet federal income (poverty) guidelines
  6. Immunization records for the child applying.
  7. Insurance card
  8. Hospital Blue Card
